Wage Range: $75,837 - $121,306 per yearPosted wage ranges represent the entire range from minimum to maximum. For jobs with more than one level, the posted range reflects the minimum of the lowest level and the maximum of the highest level. Some positions also offer additional premiums based on shift, certifications or degrees. Job offers are determined based on a candidate's years of relevant experience, level of education and internal equity.Job SummaryDevelops and coordinates the continuum of care process for the patients in the Bariatric Surgery and Weight Management Program. Duties include fostering a strong partnership between the facility and the program physicians to create a successful and comprehensive hospital-based Bariatric program with joint goals.Primary Duties:1. Partners with stakeholders to plan, develop, and implement the program and processes for the Bariatric and Weight Management Program.2. Partners to develop long-term goals and performance metrics.3. Organizes programs and activities in accordance with the mission and goals of the organization.4. Serves as point person for all certification and credentialing programs and processes.5. Works with finance and quality to develop monthly reporting processes.6. Implements and manages changes and interventions to ensure project goals are achieved.7. Acts as a resource person to the staff and a liaison to patients and their families as well as other unit personnel and medical staff.8. Functions as a physician liaison, establishing and maintaining communications with the physicians and surgeons to promote cooperation and coordination in achieving desired out omes.9. Provides Healthline with essential information regarding program details.10. Collaborates with Community Education for planning, scheduling, and facilitating community lectures and support groups.11. Collaborates closely with financial staff on the insurance approval process to ensure appropriate reimbursement for each patient.12. Develops and uses statistical and database management programs to analyze data and prepare summary narrative reports, graphs, tables, charts, and illustrations.13. Facilitates the process to ensure quality support groups are available to participants.14.
